Check this box if you do not have an Activity Card and are requesting it for the first time (you have moved to Montgomery Village in the last year and do not have an Activity Card; you have lived in Montgomery Village many years but never got an Activity Card before; your child has turned 10 and needs their first Activity Card; or you are applying for a Non-resident Pool Membership for the first time).
