Activity Card Processing Form

This form is for residents who have either lost their Montgomery Village Foundation Activity Card or are new to Montgomery Village and do not have a card yet. Please fill out the required information below in order for staff to quickly process your request. Please do not upload photos with the subject wearing sunglasses, a hat or face mask.

I Need a Replacement Card

Check this box if you are a current Montgomery Village resident and/or have lost your Activity Card. There is a $5 fee for replacement cards.

 

Once photo(s) have been uploaded and all criteria has been met, you will be contacted to pick up your Activity Card(s) at the MVF Office (please remember that photo ID and proof of residency are required to obtain your card).

I'm a New Resident/LAPM/Need a New Card

Check this box if you do not have an Activity Card and are requesting it for the first time (you have moved to Montgomery Village in the last year and do not have an Activity Card; you have lived in Montgomery Village many years but never got an Activity Card before; your child has turned 10 and needs their first Activity Card; or you are applying for a Non-resident Pool Membership for the first time).
